Payment & Insurance

We are committed to providing you with the best possible care. If you have dental or medical insurance, we want to help you receive your maximum allowable benefits as a reimbursement from your insurance company. In order to achieve this goal, we need your assistance, and your understanding of our payment policy. Please be advised that we do however require payment for oral surgical treatment when the treatment is provided. We accept personal checks, Visa, and Master Card. We do not accept American Express or the Discover Card.

We are happy to either process your insurance claim for you, or provide you with the necessary form so that you can do it yourself. We can do either of these things for you immediately after you have taken care of your consultation or surgical fee directly with us on the day those charges are incurred.

Alternatively, we can "pre-authorize" your insurance for the future surgery after you have had a consultation and taken care of the consultation fee. Dr. Strahs will determine what surgical procedure you will require and, with your permission, the office manager will furnish your insurance company with appropriate "CPT codes" to determine what your insurance company covers of the future charges incurred, if in fact they will cover the procedure. It usually takes 2 - 6 weeks for the insurance company to respond to us.

Our office is contracted or affiliated with the following PPO dental plans:

  • Assurant/DHA
  • Delta PPO and Premier (not HMO)
  • UFCW

If you are on a plan for which we are not a contracted provider, Dr. Strahs can still be your oral surgeon. You would need to take care of your charges directly with us, and we then would bill your plan for your reimbursement.

Our professional and financial relationship is with you, not your insurance company.

While the filing of insurance claims is a courtesy that we extend to our patients, all charges are ultimately your responsibility. If you have any questions about the above information, please do not hesitate to ask our office manager.
